Reimbursement Form

Before you submit a reimbursement, please confirm with Nathan Zee (PTA Treasurer) or another Board member that your expenses are covered in the PTA annual budget, and are approved for reimbursement. Please allow 7-10 days for us to review the request and provide reimbursement.

Electronic Reimbursement

  1. Take a picture of your receipts. (Recommended app: TurboScan makes it easy to scan receipts with your iPhone. Protip: it’s also great for scanning math homework to send to classmates.)
  2. Fill out 2015-2016 Reimbursement Request Form (coming soon) online. (Save it and fill it out so you can email it in the next step.) If you wish to submit an idea for a new project you can fill out a  PTA Request for funds (coming soon) PTA funds form.
  3. Email the scanned receipts and reimbursement form to treasurer@campbellschool.org.
  4. Profit!

Print Reimbursement

  1. Download a PDF reimbursement form (coming soon), print it out, fill it out by hand. Staple or clip your receipts to it.
  2. You can then either drop it in the PTA box in the front of the school, mail it to Campbell to Nathan Zee’s (PTA Treasurer) attention, or give it directly to Nathan Zee.
  3. Profit!
